Doctor Complaint: Dr Armin Shilpa - Saint Barnabas Medical Center

The document indicates that I said 'my husband verbally abused me' when it actually was a bad argument. It states I take some medication called Ultracet which I don't know anything about. It's states that I said I wanted to die when all I wanted to is 'sleep'. It states I was halllucinating/delusional when it hypocritical states 'patient is awake and alert, mentally intact but emotionally upset'. It states the I smoke which is a blatant since I am a non-smoker. It claims 'suicidal ideation, homicidal ideation, trauma, fever, headache, dyspnea, vomiting and substance abuse'. None of which is true. It says I have asthma which I don't. It doesn't mention I have MS. I have been trying to reach a Doreen Duchak (973-322-5325) who is supposed to be the head of the Emergency Department but it has been 3 days with no answer. I need this report to be cleared of the false statements and allegations and sent with the correct information.
